History
In August 2005 there was a "Culture Change Caravan" that toured the state. The Caravan was sponsored by Lumetra in partnership with: the Centers for Medicare & Medicaid Services (CMS) Region IX, the California Department of Health Services' (DHS) Licensing & Certification program, the California Ombudsman, Aging Services of California, the California Association of Health Facilities (CAHF), and the California Hospital Association (CHA).

An 18 member advisory board.

Funding
Recently received foundation funding for a staff person 80%, administered through Aging Services of California, the non-profit provider trade association.

Major Activities This Year

  • CCCC was commended in the report Improving Nursing Homes: Impact of the California Culture Change Coalition, which concluded that the "California Culture Change Coalition has played the key role in Culture change education and adoption in the Golden State."
  • Conducted six seminars throughout California attended by over 500 healthcare workers representing 125 skilled nursing facilities entitled Transform Your Nursing Home.  Our partnership with our QIO – Healthcare Services Advisory Group (HASG) allowed us to offer this valuable training and 5 CEU's for only $10 per person.
  • Promoted the Advancing Excellence Campaign webinars titled Consistent Assignment and Reducing Staff Turnover with speaker David Farrell, CCCC board member, which was attended by 489 healthcare workers from 33 states.
  • Presented at the California Association for Health Facilities (CAHF) annual conference on Are we asking the Right Questions by Darren Trisel, NHA and CCCC Vice President.
  • Presented fourteen consumer education sessions for the Pioneer Network project, entitled Creating Home: Advocating for Change in How and Where We Age, funded by The Picker Institute.
  • Conducted an Administrator Forum with seven nursing homes in attendance.
  • Presented Healing Touch at the Spa Experience to approximately 280 participants in San Diego, California at the CDPH "Rap Session."
  • Presented testimony at the California Senate Hearing convened by Senator Elaine Acquist on the topic of culture change.  David Nolan, CCCC President, as well as several other CCCC board members, testified on behalf of CCCC.
  • Participated in meetings with CDPH/OSHPD to review and revise Life Safety Codes to enable and support with resident-centered care projects.
  • Completed the final phase and report for the California Healthcare Foundation (CHCF) Grant titled Improving End of Life Care in California's Nursing Homes, which was a three-year collaborative effort with the Coalition of Compassionate Care of California to promote the CARE (Compassion and Respect at the End of life) Recommendations, Physician Orders for Life-Sustaining Treatment (POLST), and adoption using culture change principles.
  • Received a grant of $25,000 from CHCF to Develop an Operational Plan for the California Culture Change Coalition for 2012-13, which has set a foundation for the Coalition's future influence on person-centered care in California.    
  • The Coalition was commended in the report Improving Nursing Homes: Impact of the California Culture Change Coalition, which concluded that the "California Culture Change Coalition has played the key role in Culture change education and adoption in the Golden State."
  • Submitted Letter of Intent and began work on proposal for 6 Million Dollar grant from CMS Health Care Innovations Challenge.
  • Discussed and approved submittal for Tier One Coalition Partner Application to the Pioneer Network.
  • Met with new Pioneer Network CEO, Peter Reed, and discussed CA site visits and potential collaborative efforts as well as on-going invitation to coalition Board meetings.
  • Four CCCC Board members attended Pioneer Network Annual Conference.
  • CCCC began talks to assist HSAG in resident-centered care delivery models for 2012/2013.
  • Elected and expanded a new Board and officers.
  • Created new subcommittees to assist in development of a two-year work plan.
  • The volunteer board of CCCC contributed expertise and leadership while assuming operation of existing program of work which enables CCCC to build their treasury and reposition for a strong future.
  • Reviewed and updated CCCC website (www.calculturechange.org).
