History
The Modular Education Program for Activities Professionals (MEPAP Part II) selected the development of the Delaware Culture Change Coalition (aka "Trail Blazers" ) in September 2002 as a class project.
Organizational Structure
- Informal, but dedicated advisory committee consisting of long-term care professionals and advocates;
- During the 8th scope of work, partnered with the Delaware QIO;
- Mission: To transform the quality of life of everyone living and working in long-term care settings.
Funding
- Donations and sponsors for special events.
- Have initiated the process of obtaining 501 C-3 status to access grant and other funding sources.
Major Activities This Year
- Participated in and assisted with planning and organizing several conferences and workshops with national and state speakers.
- Maintain a comprehensive lending library of books, videos and other educational materials.
- Provided guest speakers, advisors and support to other states.
- Continued to educate residents and family members about culture change and resident rights.
- Participated in CMS/Pioneer Network symposium on culture change and the physical environment requirements as well as the 2008 Pioneer Network conference.
