History
Began meeting in January 2005 in Columbia, Missouri and decided to meet quarterly thereafter.Local QIO (Primaris) and individuals involved in culture change activity in their jobs were a driving force in initial meetings. Providers have increased attendance at quarterly meetings and Regional groups have become active in 2006. By 2007, eight regional groups had been formed and were meeting across the state. Culture Change Conferences were held in partnership with MOAHA in 2007 and 2008.
Organizational Structure
- Organizational Structure consists of: a Steering Committee including Chair, Co-Chair, Education Task Force Leader, Communication Task Force Leader, and Regional Representatives;
- Currently the coalition has 8 regional groups: East, West, West Central, Central, South Central, Southeast, Northwest, and Southwest;
- Working on changing Steering Committee to Board of Directors and becoming a 501 C-3 Organization;
- Currently members of the Steering Committee come from the LTC Ombudsman Program, the Alzheimer's Association, DHSS, not for profit and for profit providers, and the QIO.
Funding
No funding at present. Member organizations such as state QIO (Primaris) have donated services such as maintenance and use of a listserv, use of their facility to meet and donation of staff time. Donations have been solicited and accepted from member organizations to help fund website setup and ongoing maintenance. Some funds have been generated from yearly conference held in partnership with MOAHA. Currently pursuing becoming a 501 C-3 and then plan to apply for several grants.
Major Activities This Year
- Missouri and MC5 were host to the 2011 Pioneer Network Conference themed "Explore, Discover, Change" held in St. Charles, Missouri in August. There were over 350 participants from Missouri, including approximately 70 state employees.
- MC5 sold approximately 300 t-shirts with the theme "Show Me
culture change" at the Conference with proceeds going to support further culture change projects throughout Missouri.
- MC5's Education Committee implemented a lending library in collaboration with the Alzheimer's Association St Louis Chapter and the Long Term Care Ombudsman Office.
- MC5 put technology to work for our providers, hosting two (2) statewide webinars.
- The Artifacts of Culture Change (February, 2011) was available to and attended by all regions. Carmen Bowman was the educator.
- The Risk of Safety, presented by Ray Miller, was held "live" in Osage Beach and viewed in St. Louis and Kansas City (April 2011). The event also included an expert panel with representatives from the Missouri Department of Health and Senior Services, University of Missouri Sinclair School of Nursing, and the Administrator from a Missouri household model long term community.
- MC5 Web site was given a new look
- Sam Plaster, Advisory Board Member to MC5, Julie Ballard, Vice-President of the Board and Zoe Dearing, Education Committee Chair organized and sponsored training for the Regional Group Leaders in order to support the growth of the regional groups.
- Began work in collaboration with the Missouri LANE and other provider groups on educational programs supporting quality of care with a focus on person centered care.
